Our staff are ready to help you fill out needed paperwork. We don't want forms to be a barrier to receiving services.

Get a diagnostic report from a psychologist documenting an autism diagnosis
Submit insurance information & we check benefits and eligibility
Complete intake form
Complete release of information form so we can collect needed documents from psychologists or school
Submit the diagnostic report to us
We submit request to insurance to assess needs of child
We assess needs of child when we receive authorization
We write a treatment plan and submit to insurance for approval
Start treatment when insurance approves treatment plan
